“Fixed dose combination pills are particularly useful in India because patients are less likely to adhere to multiple individual drugs owing to the cost factor,” says Dr. MK Das in an interview with Dr. Rishi Jain “It is preferable to achieve the target blood pressure with mono therapy but if the target is not reached by more than 2/10 mmHg, then American Heart Association and other societies recommend combination therapy to achieve the target faster, thereby reducing adverse cardiovascular events.” “In a fixed drug combination, doctors should have a free hand in adjusting the dose for the best results,” says Dr. Das to address the challenges in titration. Regarding the poly pill therapy, Dr. MK Das says, “The concept of poly pill is an extension of the fixed dose combination therapy. A poly pill, in addition to hypertension, can take care of other diseases like metabolic syndrome, dyslipidemia, and coronary artery disease.” “TIPS 1 and TIPS 2 trials show that although poly pills have reduced risk factors like hypertension, they haven’t shown a mortality benefit. TIPS 3 has been initiated to improvise on the results, which should come out by 2016-17.”
