Vascular trauma is a very special type of problem and the whole principle is to try and limit the ischemic time and we must not forget fasciotomy. You can perform early fasciotomy before the orthopedic surgeons decide to stabilize a fracture because it is no good trying to do complex vascular reconstruction if the bones are destabilized because of fracture.
