Mechanical Thrombectomy and the 24-Hour Treatment Window

Text Size:
The evolution of Endovascular Therapy (EVT) has radically transformed the prognosis for patients suffering from Large Vessel Occlusions (LVOs). Before landmark trials like DAWN and DEFUSE 3, patients arriving beyond the 6-hour window were largely consigned to supportive care and severe permanent disability. In 2026, the combination of advanced stent retrievers, large-bore aspiration catheters, and perfusion imaging has pushed the intervention window up to 24 hours for carefully selected patients.
"Care for stroke is an innovative intervention which continues to prove its immense clinical and cost-effectiveness through rigorously designed clinical trials," states Professor Jeyaraj Pandian, President of the World Stroke Organization.
The procedure involves a neuro-interventionalist navigating a catheter from the femoral or radial artery directly into the brain's blocked vessel—most commonly the Middle Cerebral Artery (MCA) or Internal Carotid Artery (ICA)—to physically extract the thrombus. When performed successfully, EVT can instantly restore cerebral blood flow, allowing patients who presented with severe paralysis and aphasia to walk out of the hospital days later.
"Knowing the symptoms and signs of a stroke and acting fast remains critical, as endovascular therapies are highly time-dependent, even with expanded windows," emphasizes Professor Sheila Martins, Immediate Past President of the World Stroke Organization.
Despite its miraculous efficacy, access to EVT remains heavily restricted by geography. Expanding the network of Comprehensive Stroke Centers equipped with 24/7 neuro-interventional suites is the most pressing infrastructural mandate for modern healthcare systems globally.
A fake news pandemic has taken the world by storm. More so for medical content.
We are witnessing a paradox of information overload and unanswered questions among both the lay public and the medical community.
TheRightDoctors brings you Insights from thought leaders across leading medical specialities. Views which help correct the information asymmetry and articulate the voice of medical science and reason.
This effort, however, needs your support.
