Obicetrapib Cuts LDL Cholesterol by 29.9% in Patients at High Risk for Cardiovascular Events, NEJM Study Finds

A pioneering study published in the NEJM on May 7, 2025, revealed that obicetrapib, a selective cholesteryl ester transfer protein (CETP) inhibitor, significantly reduces low-density lipoprotein (LDL) cholesterol levels in patients at high risk for cardiovascular events.
This discovery offers hope for better management of atherosclerotic cardiovascular disease (ASCVD) and heterozygous familial hypercholesterolemia (HeFH).
As the global trial highlights, “the research addresses a critical need for effective therapies in patients already on maximum lipid-lowering treatments.”
According to the BROADWAY trial, “2,530 patients, with a mean age of 65, including 34% women, were enrolled.”
The investigators observed a mean baseline LDL cholesterol level of 98 mg per deciliter in these patients.
Those administered 10 mg of obicetrapib daily achieved a 29.9% reduction in LDL cholesterol by day 84, compared to a 2.7% increase in the placebo group—resulting in a between-group difference of 32.6 percentage points (P<0.001), as per the trial data.
“Adverse events were reported to be similar in both groups, indicating a favorable safety profile,” the researchers reported.
The study concluded that “among patients with ASCVD or HeFH receiving maximum tolerated doses of lipid-lowering therapy, obicetrapib reduced LDL cholesterol levels by 29.9%.”
“This represents a significant advancement for high-risk patients unable to meet stringent LDL targets,” according to experts.
“The mechanism of obicetrapib involves inhibition of CETP, a protein that facilitates cholesterol ester transfer among lipoproteins,” as detailed in the study.
By blocking this transfer, obicetrapib reduces LDL cholesterol—commonly referred to as “bad cholesterol”—and may also preserve or elevate high-density lipoprotein (HDL) cholesterol, known as “good cholesterol,” researchers explained.
“This dual lipid-modifying action offers a potential reduction in cardiovascular events and a convenient oral alternative to injectables like PCSK9 inhibitors,” the findings imply.
A 2022 study published in Nature Medicine showed that obicetrapib reduced LDL cholesterol by up to 51% in dyslipidemic patients on high-intensity statins, supporting its clinical promise.
“Unlike previous CETP inhibitors such as torcetrapib, which were discontinued due to safety concerns, obicetrapib appears to be well-tolerated,” according to the investigators.
This development is particularly impactful for millions with ASCVD or HeFH who remain above LDL targets despite aggressive treatment, as per the authors.
Experts believe that “obicetrapib could become a vital part of combination therapy regimens, meeting unmet needs in cardiovascular care.”
